Notice of Privacy Practices

The complete Notice of Privacy Practices is provided to every patient at registration, and you are asked to acknowledge receipt of it then.

You may also request a copy at any time, before or after your first visit, by calling (818) 290-3711, by writing to contact@rejuve.com, or by asking at reception. We will provide it in paper or electronically, as you prefer, at no charge.

We are required by law to maintain the privacy of your protected health information, to give you notice of our legal duties and privacy practices with respect to that information, and to notify you if a breach of your unsecured protected health information occurs.

We are required to follow the terms of the Notice currently in effect. We may change it, and any change applies to information we already hold as well as to information we receive afterwards.

Under the HIPAA Privacy Rule you have the right to:

• Inspect and obtain a copy of your medical and billing records, in paper or electronically
• Ask us to correct health information you believe is incorrect or incomplete
• Ask us to limit the health information we use or disclose for treatment, payment or healthcare operations
• Ask us to contact you in a particular way, or at a particular address
• Receive a list of the disclosures we have made of your health information
• Obtain a paper copy of the Notice, even if you agreed to receive it electronically
• Withdraw a written authorisation you have given, except where we have already acted on it
• Complain to us or to the U.S. Department of Health and Human Services if you believe your rights have been violated, without being penalised for doing so

If you believe your privacy rights have been violated, you may complain to us in writing at Rejuve Clinics Sherman Oaks, Inc., 15301 Ventura Blvd, Suite U2, Sherman Oaks, CA 91403, or to the Office for Civil Rights at the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
